A client books a peel and mentions in passing that she is “taking something for acne”. If it is isotretinoin, the peel has to wait — provided somebody asks. Below you will find a core beauty consultation form (in Polish salons, the wywiad kosmetyczny), extra questions for five treatment groups and how to collect the answers before the visit.
What a beauty consultation form should cover
- Health — conditions that change the treatment plan or rule it out.
- Medication and supplements — especially retinoids, antibiotics, anticoagulants and photosensitising drugs.
- Allergies, pregnancy and breastfeeding.
- Home care and previous treatments — where to start and what to avoid.
- Expectations — what the client wants to improve.
- Declaration and consent — that the answers are true, plus explicit consent to processing health data.
Do the full consultation before the first treatment and repeat it once a year, after a change of medication, during pregnancy and after giving birth, after a serious illness and before a new type of treatment. At every other visit a quick “Has anything changed since last time — medication, health, pregnancy, tanning?” is enough, with the answer and a signature going into the record.

How to do it in Terminka
You don’t build a consultation form in the app from scratch — the core questions are already there, and you add your treatment block yourself.
- Take a ready-made template. In Client card settings → Card templates → Ready-made templates, each of the 31 templates has the same fields: main concern or goal, allergies, medication and supplements, contraindications and health notes, skin reaction history (none / mild redness / strong sensitivity), pregnancy or breastfeeding, last similar treatment, a before photo and treatment notes. “Cosmetology consultation” is a good place to start.
- Add your treatment block. Tap Create based on this and add questions with Add field: yes/no questions as Yes / No, lists as Single choice or Multiple choice, dates as Date, everything else as Text. Choose the type as the app’s hint describes it: Profile — once per client, Treatment — every visit. A completed consultation stays current with no expiry; to have it come back to the client, say, once a year, set Card validity to 12 mo. Also check that Health data (sensitive) is ticked.
- Send the link before the visit. Attach the record to a service (Auto-suggest on booking → Services), and the visit’s Documents and care block shows what is missing. You send the link yourself — SMS link or Email link; nothing is sent automatically. The client fills in the form in her browser, with no app, and has time to look up the names of her medicines.
- The client signs. With her finger on the screen or with an SMS code; before signing she can see who the controller of her data is. A ready-made explicit consent to health data (art. 9 GDPR) is in the Consents tab.
- You read the answers before the treatment. Under Client info → Card → Cards the consultation is marked Completed. Access to records depends on each team member’s role, and opening health data is recorded in the access log.

How to ask without it feeling like an interrogation
- Say why you are asking. “I’m asking about medication because it decides which product I can safely use” lands very differently from a curt “medication?”.
- Closed questions first, details after a “yes”. The form is quicker to fill in and nothing slips through.
- Sensitive questions in writing, not out loud at a busy reception desk.
- Don’t judge. A sunbed session a week before laser is information, not a reason for a lecture — a client who feels judged will leave something out next time.
Frequently asked questions about consultation forms
Do I need the client’s consent for the consultation?
Information about illnesses, medication, allergies or pregnancy is health data — a special category of personal data (art. 9 GDPR, known in Poland as RODO). Salons usually collect explicit consent for it: separate, not buried in the salon terms and not bundled with marketing. Before the consultation the client should also know who the data controller is, why you collect the data, how long you keep it and what her rights are. Don’t ask “just in case” about things unrelated to your treatments.
What if the client doesn’t want to answer?
She has every right not to. Don’t push — note in the record which question she skipped. If the treatment can’t be done safely without that information, say so plainly and offer a different treatment or another day. Treating “blind” is a risk that no signature takes off your shoulders.
